目的探討腘窩中間動脈蒂混合供血皮瓣的血供特點及修復兒童腘窩瘢痕攣縮松解后創(chuàng)面的可行性。 方法2008年1月-2010年10月,對6例腘窩瘢痕攣縮患兒行瘢痕松解、腘窩中間動脈蒂混合供血皮瓣移位修復。男2例,女4例;年齡2歲2個月~10歲。均為熱水燙傷。腘窩瘢痕攣縮松解后創(chuàng)面范圍5 cm × 4 cm~10 cm × 8 cm,皮瓣切取范圍6 cm × 4 cm~11 cm × 9 cm。供區(qū)直接縫合1例,游離植皮5例。 結果術后6例患兒皮瓣完全成活,無血管危象或皮瓣壞死發(fā)生。供、受區(qū)切口均Ⅰ期愈合,供區(qū)植皮均成活。術后患兒均獲隨訪,隨訪時間12~24個月,皮瓣色澤、質地、外形良好。1例患兒術后6個月腘窩手術切口出現較厚增生性瘢痕,膝關節(jié)活動范圍0~175°,隨訪15個月無明顯變化;余5例膝關節(jié)活動范圍0~180°,無跛行。 結論腘窩中間動脈蒂混合供血皮瓣血供可靠,手術操作簡便,修復兒童腘窩瘢痕攣縮松解后創(chuàng)面效果理想。
