目的  系統(tǒng)評(píng)價(jià)腎移植術(shù)后免疫抑制劑驍悉(mycophenolate mofetil,MMF)與硫唑嘌呤(azathioprine,AZA)導(dǎo)致的不良反應(yīng)發(fā)生情況.
方法  計(jì)算機(jī)檢索MEDLINE(1966~2004.6)、EMBASE(1984~2004.6)、Cochrane圖書(shū)館(2004第2期)及中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù)(1979~2004.6),納入涉及腎移植術(shù)后使用MMF與AZA出現(xiàn)不良反應(yīng)的隨機(jī)對(duì)照試驗(yàn)(RCT),對(duì)其方法 學(xué)質(zhì)量進(jìn)行評(píng)價(jià)后,采用RevMan 4.11軟件進(jìn)行統(tǒng)計(jì)分析.
結(jié)果  共納入24個(gè)RCT,涉及不同劑量MMF與AZA治療腎移植術(shù)后的排斥反應(yīng).MMF的胃腸道不良反應(yīng)發(fā)生率明顯高于AZA,特別是MMF3 g/d組的嘔吐、腹痛、腹瀉發(fā)生率與AZA組比較有明顯差異(P lt;0.05).MMF3 g/d組的巨細(xì)胞病毒感染發(fā)生率在6個(gè)月、1年及3年觀察期內(nèi)明顯高于AZA組,其差異有統(tǒng)計(jì)學(xué)意義;MMF2 g/d組在1年隨訪期內(nèi)感染發(fā)生率也高于AZA組.MMF3 g/d組的白細(xì)胞減少發(fā)生率明顯高于AZA組,其差異有統(tǒng)計(jì)學(xué)意義;而MMF2 g/d組的白細(xì)胞減少發(fā)生率與AZA組相比差異不具統(tǒng)計(jì)學(xué)意義.MMF的血小板減少發(fā)生率低于AZA組,MMF3 g/d組與AZA組相比差異有統(tǒng)計(jì)學(xué)意義.MMF各組及AZA組皮膚腫瘤的發(fā)生率無(wú)明顯差異.
結(jié)論  MMF與AZA相比,其不良反應(yīng)發(fā)生主要表現(xiàn)為較高的胃腸道不良反應(yīng)、白細(xì)胞減少發(fā)生率和機(jī)會(huì)感染發(fā)生率,較低的血小板減少發(fā)生率.而且不良反應(yīng)的發(fā)生呈現(xiàn)一定的劑量效應(yīng)關(guān)系.
