目的:探討18F-FDG PET/CT在頭頸部腫瘤的臨床應(yīng)用價值。方法:58例頭頸部惡性腫瘤病例,男37例,女21例,年齡21~78歲。其中:牙齦癌3例,上頜竇癌2例,舌癌2例,腮腺癌1例,鼻咽癌24例,喉癌8例,甲狀腺癌4例,原發(fā)灶不明的頸部淋巴結(jié)轉(zhuǎn)移瘤14例。使用18F-FDG行全身PET/CT掃描,依據(jù)PET圖像、CT圖像和PET/CT融合圖像及標(biāo)準(zhǔn)化攝取值(SUV)進行綜合評價。結(jié)果:29例放療患者中的11例擬行根治性放療的患者,有4例改行姑息性放療,8例重新勾畫了放療靶區(qū)及調(diào)整了放療劑量,3例改行其它治療;15例進行了放療后的療效評估;14例原發(fā)灶不明的頸部淋巴結(jié)轉(zhuǎn)移瘤8例找到了原發(fā)灶。結(jié)論:PET/CT可以對頭頸部惡性腫瘤進行準(zhǔn)確的臨床分期,精確勾畫放療的生物靶區(qū),準(zhǔn)確而快捷地確定腫瘤復(fù)發(fā)的位置與侵犯范圍,在頸部不明原發(fā)灶轉(zhuǎn)移瘤的應(yīng)用中具有簡便、快捷、無創(chuàng)和靈敏等臨床特點。
