• 揚州大學(xué)臨床醫(yī)學(xué)院肝膽外科（江蘇揚州 225001）;

目的 　探討經(jīng)臍單孔腹腔鏡膽囊切除（LC）治療小兒膽囊良性疾病的可行性、手術(shù)方法及臨床應(yīng)用價值。
方法 　回顧性分析2009年6月至2011年6月期間我院收治的64例小兒LC患者的臨床資料，其中行經(jīng)臍單孔LC手術(shù)41例（經(jīng)臍單孔LC組），行常規(guī)三孔LC手術(shù)23例（三孔LC組）。記錄2組患者的手術(shù)時間、術(shù)中失血量、中轉(zhuǎn)情況、并發(fā)癥及住院時間；運用視覺模擬評分法（VAS）對2組患者術(shù)后3h、6h、12h、24h、48h及72h的疼痛進行評分；采用Kiyak滿意度問卷對2組患者術(shù)后1周、2周、1個月、3個月、6個月及12個月的滿意度進行評分，并做統(tǒng)計學(xué)分析。術(shù)后隨訪12個月。
結(jié)果 　64例患者均順利完成手術(shù)，無中轉(zhuǎn)開腹病例，僅經(jīng)臍單孔LC組有2例增加了戳孔。2組均無膽管損傷、膽汁漏及切口疝發(fā)生。三孔LC組有1例發(fā)生切口感染 〔4.35%（1/23）〕，經(jīng)臍單孔LC組發(fā)生1例切口感染和1例皮下血腫 〔4.88%（2/41）〕，2組總的并發(fā)癥發(fā)生率比較差異無統(tǒng)計學(xué)意義（P＞0.05）。經(jīng)臍單孔LC組和三孔LC組在手術(shù)時間〔（47.54±18.71） min比（45.33±10.58） min〕、術(shù)中失血量〔（18.56±13.34） ml比（17.28±12.53） ml〕及住院時間〔（1.67±0.36） d比（1.81±0.38） d〕方面比較差異均無統(tǒng)計學(xué)意義（P＞0.05）。2組VAS疼痛評分在術(shù)后24h內(nèi)比較差異均無統(tǒng)計學(xué)意義（P＞0.05），術(shù)后24h后經(jīng)臍單孔LC組VAS疼痛評分較三孔LC組明顯降低（P＜0.05）。2組術(shù)后隨訪12個月，經(jīng)臍單孔LC組不同隨訪時間點滿意度評分均明顯高于三孔LC組（P＜0.05）。
結(jié)論 　經(jīng)臍單孔LC手術(shù)能有效緩解術(shù)后疼痛，美容效果明顯，患者滿意度高，治療小兒膽囊良性疾病安全、可行。
