目的  探討改良Glisson肝蒂法在肝癌患者肝切除術(shù)中應(yīng)用的可行性。
方法  對60例肝切除術(shù)后病理學(xué)檢查證實(shí)的肝細(xì)胞癌患者進(jìn)行分組研究。實(shí)驗(yàn)組(Glisson組)經(jīng)過術(shù)前精確評估后,術(shù)中采取改良Glisson肝蒂法首先阻斷肝段肝蒂,然后按照術(shù)中B超定位肝靜脈主干及分支作為肝內(nèi)斷肝平面,采取精細(xì)鉗夾法切除單一以及聯(lián)合肝段。對照組(Prigle組)行常規(guī)術(shù)前檢查及評估,術(shù)中采取傳統(tǒng)Prigle法阻斷第一肝門后,以超聲刀不顯露肝靜脈按照術(shù)中B超定位平面切除單一或聯(lián)合肝段。
結(jié)果  術(shù)中出血量、輸血量、手術(shù)時(shí)間和術(shù)后肝功能指標(biāo)、術(shù)后恢復(fù)時(shí)間及術(shù)后白蛋白用量2組間差異均無統(tǒng)計(jì)學(xué)意義(P>0.05); Glisson組術(shù)后并發(fā)癥發(fā)生率(23.3%)低于Prigle組(50.0%),P<0.05;Glisson組術(shù)后病理標(biāo)本切緣距腫瘤距離為(2.3±0.7) cm ,大于Prigle組的(1.5±0.6) cm,P<0.05。Glisson組術(shù)后1年的腫瘤復(fù)發(fā)率略低于Prigle組〔10.0%(3/30)比16.7% (5/30)〕,但差異沒有統(tǒng)計(jì)學(xué)意義(P>0.05)。
結(jié)論  改良Glisson肝蒂法肝切除在肝癌患者中的應(yīng)用具有與傳統(tǒng)Prigle法同樣的安全性,且可減少術(shù)后并發(fā)癥的發(fā)生,保證手術(shù)切緣,同時(shí)理論上可以降低術(shù)后復(fù)發(fā)率。
