目的 　回顧性分析88例胰島素細(xì)胞瘤病例，探討胰島素細(xì)胞瘤的臨床表現(xiàn)、診斷及治療方法，總結(jié)出對于胰島素細(xì)胞瘤的診治經(jīng)驗。
方法 　對筆者所在醫(yī)院2003年10月至2010年1月期間收治的88例胰島素細(xì)胞瘤患者的臨床資料進(jìn)行回顧性分析。
結(jié)果 　88例病例中女性患者占71.6% （63/88），年齡（38.59±11.95）歲，體質(zhì)量指數(shù)為27.78±5.86，腫瘤直徑為（1.62±0.70） cm，其中12例為多發(fā)性。Whipple三聯(lián)征及空腹胰島素/葡萄糖比值對胰島素細(xì)胞瘤定性診斷的敏感度分別為86.4% （76/88）和79.5% （70/88）。腹部超聲、CT、MRI及術(shù)中超聲檢查對胰島素細(xì)胞瘤定位診斷的敏感度分別為30.8% （24/78）、74.6% （53/71）、82.5% （47/57）和100% （59/59）。88例患者中，僅行腫瘤剜除術(shù)60例，僅行胰腺遠(yuǎn)端切除術(shù)24例，同時行腫瘤剜除術(shù)及胰腺遠(yuǎn)端切除術(shù)4例。行腫瘤剜除術(shù)及胰腺遠(yuǎn)端切除術(shù)后胰瘺發(fā)生率分別為37.5% （24/64）和14.3%（4/28），總住院時間和術(shù)后住院時間分別為28d及16d和29d及13d，其差異均無統(tǒng)計學(xué)意義（P＞0.05）。
結(jié)論 　對胰島素細(xì)胞瘤可根據(jù)Whipple三聯(lián)征以及空腹胰島素/葡萄糖比值獲定性診斷，定位診斷可依靠術(shù)中超聲。禁食試驗可縮減至15h。腫瘤剜除術(shù)與胰腺遠(yuǎn)端切除術(shù)的治療效果及并發(fā)癥發(fā)生率無明顯差異。
