目的 　總結(jié)分析殘胃癌的臨床病理學(xué)特征。
方法 　回顧性分析2000年3月至2008年5月期間川北醫(yī)學(xué)院附屬第二醫(yī)院和中國人民解放軍總醫(yī)院共114例殘胃癌的臨床病理學(xué)資料。依據(jù)原發(fā)疾病性質(zhì)分為良性疾病組和惡性疾病組進行分析。
結(jié)果 　本組114例患者，年齡（62.6±11.3）歲，男∶女=4.7∶1.0。76.2% （64/84）的患者診斷時處于進展期（以pT分期為依據(jù)），pT1期病例僅占23.8%（20/84），60.7%（51/84）的病例達pT4期。殘胃癌的一般病理特征：pT4b（27.4%，23/84）、pN＋（42.9%，36/84）及cM1（27.2%，31/114）常見，93.5%（29/31）的遠處轉(zhuǎn)移位于腹腔，腹膜轉(zhuǎn)移最高達67.7%（21/31）。組織學(xué)分化程度以低分化腺癌最常見（76.7%，79/103），Laurén分型以彌漫型最常見（78.6%，81/103）。良、惡性原發(fā)疾病間首次采用的手術(shù)方式和重建方式差異均有統(tǒng)計學(xué)意義（均P=0.000）。中位發(fā)病時間（潛伏期）良性組∶惡性組=30.0年∶3.3年（P=0.000），主要受原發(fā)疾病性質(zhì)、首次手術(shù)時年齡的影響（P＜0.05）。良、惡性組間的病理學(xué)指標(biāo)比較，除是否為印戒細胞癌（P=0.045）、是否侵犯周圍臟器（P=0.049）、pN分期（P=0.025）及Borrmann分型（P=0.005） 差異有統(tǒng)計學(xué)意義外，其余病理學(xué)指標(biāo)以及手術(shù)切除率、R0切除率和生存率差異均無統(tǒng)計學(xué)意義（P＞0.05）。
結(jié)論 　殘胃癌的主要臨床病理學(xué)特征基本上不受原發(fā)疾病性質(zhì)的影響，建立個體化的內(nèi)鏡隨訪程序是必要和可行的。
