目的 探討胰腺胃泌素瘤的診斷和治療方法。
方法 回顧性分析1990年1月至2011年12月期間海南省人民醫(yī)院收治的13例胰腺胃泌素瘤患者的臨床資料。術前根據(jù)臨床表現(xiàn)及實驗室檢查結果進行定性診斷，以腹部彩色多普勒超聲、CT等檢查進行定位診斷。術后根據(jù)病理學結果給予相應的放化療。
結果 13例患者均行手術治療。術中發(fā)現(xiàn)腫瘤位于胰頭者9例，胰尾者1例，胰體者2例以及胰腺內多發(fā)腫瘤者1例； 腫瘤直徑為0.2～4.0cm，其中＞2.0cm者11例。行胰體尾＋脾切除術1例，行瘤摘除術7例，行膽胰結合部切除術2例，行胰十二指腸切除術3例。術后出現(xiàn)胰液漏1例，切口感染2例。13例中有12例獲得隨訪，隨訪時間16～120個月，平均隨訪時間為78個月。12例胰腺胃泌素瘤患者術后腹瀉等臨床癥狀消失；服用質子泵抑制劑治療，半年后行電子胃鏡檢查，顯示潰瘍均愈合，基礎胃酸分泌及12h胃液量均在正常范圍內。1例因其他疾病于術后1年死亡；1例因腫瘤復發(fā)于1.5年后死亡，另1例因肝轉移行再次手術治療4年后死亡；其余10例痊愈。
結論 外科治療是胰腺胃泌素瘤有效的治療方法。
