目的  總結(jié)腹腔鏡下恥骨上切口疝修補(bǔ)的手術(shù)方法及效果。
方法  回顧性分析2007年3月至2010年10月期間筆者所在科室25例行腹腔鏡下恥骨上切口疝修補(bǔ)術(shù)患者的臨床資料。男13例,女12例;年齡35~83歲,中位年齡52歲;其中2例是復(fù)發(fā)疝。疝缺損下緣距恥骨弓距離均小于5cm。在直視下采用螺旋釘槍將補(bǔ)片固定到腹壁上,補(bǔ)片下邊緣須低于恥骨弓2cm并將其固定到恥骨弓及雙側(cè)的恥骨梳韌帶上。
結(jié)果  25例患者均成功完成腹腔鏡下切口疝修補(bǔ),無(wú)中轉(zhuǎn)開(kāi)腹。疝缺損最大徑為6.1~12.5cm,平均9.5cm。手術(shù)時(shí)間為90~180min,平均128min??偛l(fā)癥發(fā)生率為28%(7/25)。包括術(shù)中膀胱損傷1例;術(shù)后補(bǔ)片上方漿液腫4例,經(jīng)局部穿刺抽吸后消失;術(shù)后修補(bǔ)區(qū)域腹壁疼痛2例,未行特殊處理于術(shù)后1個(gè)月內(nèi)逐漸緩解并消失。22例患者獲隨訪,隨訪率為88%,隨訪時(shí)間為6~48個(gè)月,平均30個(gè)月,1例于術(shù)后2個(gè)月復(fù)發(fā),復(fù)發(fā)率為4%。
結(jié)論  腹腔鏡下恥骨上切口疝修補(bǔ)是一種安全有效的方法,其網(wǎng)片重疊缺損邊緣至少超過(guò)5cm,網(wǎng)片下邊緣必須在直視下固定到雙側(cè)恥骨梳韌帶上,這樣可以增加固定的強(qiáng)度并降低修補(bǔ)術(shù)后復(fù)發(fā)率。
