目的 　探討術(shù)中超聲引導(dǎo)下精準(zhǔn)Tru-cut活檢在胰頭實(shí)性腫塊病理學(xué)診斷中的價(jià)值。
方法 　對(duì)徐州醫(yī)學(xué)院附屬宿遷醫(yī)院肝膽胰脾外科2010年8月至2011年8月期間收治的胰頭實(shí)性腫塊患者行術(shù)中超聲引導(dǎo)下精準(zhǔn)Tru-cut活檢，共28例患者，男20例，女8例，年齡 34～78歲，平均 64歲； 腫塊最大徑≤1.5cm者5例（0.8～1.5cm ，平均1.2cm，S組），1.5cm＜腫塊最大徑≤3.0cm者7例（1.6～3.0cm，平均2.5cm，M組），腫塊最大徑＞3.0cm者16例（3.2～6.8cm ，平均4.8cm，L組），每一腫塊活檢3針，每一條組織條均行快速冰凍檢查，其結(jié)果與術(shù)后大標(biāo)本石蠟切片病理結(jié)果分別比較。
結(jié)果 　以患者為觀察單位，所有病例診斷符合率為100%，無(wú)假陽(yáng)性及假陰性。以單個(gè)組織條病理結(jié)果為觀察單位，僅1條符合（1/3）、僅2條符合（2/3）和3條全符合（3/3）者分別為：S組是3例 （3/5）、2例 （2/5）、0 例（0/5）；M組是0例 （0/7）、5例 （5/7）、2例 （2/7）；L組是0例 （0/16）、4例（4/16）、12例 （12/16）； 在小病灶組（S組＋M組）中的單條組織標(biāo)本的假陰性率高于L組（χ2=9.833，P=0.002）。病灶小易出現(xiàn)單次穿刺假陰性結(jié)果。
結(jié)論 　術(shù)中超聲引導(dǎo)下精準(zhǔn)Tru-cut活檢能夠準(zhǔn)確確定胰頭腫塊的病理學(xué)診斷，安全有效，可為手術(shù)決策提供可靠依據(jù)。
