目的 　探討良性胸骨后甲狀腺腫經(jīng)頸入路切除的可行性與安全性并總結(jié)手術(shù)技巧。
方法 　對我院2002年8月至2011年10月期間的85例良性胸骨后甲狀腺腫患者的臨床資料進(jìn)行回顧性分析。
結(jié)果 　85例患者均經(jīng)頸入路行胸骨后甲狀腺腫切除，未發(fā)生圍手術(shù)期死亡及呼吸道梗阻，無大出血、氣胸、血胸及乳糜漏，未發(fā)生永久性喉返神經(jīng)損傷及永久性甲狀旁腺功能低下。3例患者術(shù)后1～3d出現(xiàn)聲音嘶啞，對癥處理后恢復(fù)正常。4例患者術(shù)后1～2d出現(xiàn)四肢末端麻木，1例患者出現(xiàn)輕度抽搐，予以靜脈及口服補(bǔ)鈣治療3～7d后癥狀緩解。術(shù)后頸部引流量（68±4.9）ml （35～220ml），2～6d（平均4.2d）拔除引流管。術(shù)后2年82例患者得到隨訪，無低鈣、低甲狀旁腺激素的病例，無聲音嘶啞及局部甲狀腺腫復(fù)發(fā)的病例，2例患者出現(xiàn)甲狀腺功能低下，給予調(diào)整口服甲狀腺素劑量后恢復(fù)正常。
結(jié)論 　墜入型良性胸骨后甲狀腺腫經(jīng)頸入路切除是安全、可行的，而充分的暴露、循序漸進(jìn)的操作、尋準(zhǔn)間隙的鈍性分離和托拽結(jié)合是保證手術(shù)成功的關(guān)鍵。
