目的探討術(shù)中預(yù)防性留置皮下負(fù)壓引流管在防治肥胖患者腹部縱切口術(shù)后脂肪液化的必要性及臨床價(jià)值。方法前瞻性選擇2008年2月至2010 年10月期間洛陽(yáng)市第一人民醫(yī)院普外科和婦產(chǎn)科的342例肥胖腹部縱行切口患者，按隨機(jī)數(shù)字表法隨機(jī)分為置管組及無(wú)管組，比較２組間切口甲級(jí)愈合率、脂肪液化發(fā)生率、切口并發(fā)癥發(fā)生率、切口平均愈合時(shí)間、患者滿意度、局部異物感及直接醫(yī)療成本。結(jié)果根據(jù)納入和排除標(biāo)準(zhǔn)，共328例患者納入本研究，其中置管組165例，無(wú)管組163例。2組患者在年齡、性別構(gòu)成、脂肪厚度、手術(shù)時(shí)間、切口長(zhǎng)度及BMI方面差異均無(wú)統(tǒng)計(jì)學(xué)意義(P gt;0.05)。置管組與無(wú)管組脂肪液化發(fā)生率分別為3.6%(6/165)和5.5%(9/163)，P gt;0.05; 異物感分別為50.3%(83/165)及3.7%(6/163)，P lt;0.01; 患者滿意度分別為(6.8±1.7)分及(9.2±2.8)分(P lt;0.05)。甲級(jí)愈合率、切口并發(fā)癥發(fā)生率、切口平均愈合時(shí)間及直接醫(yī)療成本2組間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P gt;0.05)。結(jié)論對(duì)肥胖患者腹部縱切口術(shù)中不必常規(guī)放置負(fù)壓引流管預(yù)防切口脂肪液化。
