• 同濟大學(xué)附屬東方醫(yī)院微創(chuàng)外科（上海 200120）;

目的探討臨床應(yīng)用內(nèi)鏡逆行胰膽管造影術(shù)（ERCP）的診治指征。方法回顧性分析2005年1月至2009年12月期間我院采用ERCP方法進行診斷及治療的患者的臨床資料。結(jié)果221例行ERCP的患者中，術(shù)中發(fā)現(xiàn)膽總管結(jié)石99例，占45%； 惡性腫瘤44例，占20%； 乳頭狹窄9例，占4%； 陰性45例，占20%； 操作失敗24例，占11%。 年度分類統(tǒng)計顯示，行ERCP的患者數(shù)逐年減少。ERCP術(shù)后并發(fā)癥發(fā)生率為11%（25例），包括術(shù)后胰腺炎15例、出血3例、膽道感染5例及網(wǎng)籃頭滯留2例。結(jié)論在影像學(xué)及腹腔鏡技術(shù)高度發(fā)展的時代，應(yīng)本著無創(chuàng)、微創(chuàng)的原則選擇診治方法，臨床上不能無限擴大ERCP的應(yīng)用指征。
