目的探討鈣化在非腫塊型乳腺腫瘤X線診斷中的意義,提高對非腫塊型乳腺良、惡性腫瘤的鑒別診斷水平。方法回顧性分析2008年11月至2011年3月期間四川大學華西醫(yī)院經病理檢查證實的X線片未見腫塊的90例乳腺腫瘤患者的臨床資料,其中乳腺良性腫瘤55例,乳腺癌35例,觀察并記錄鈣化的形態(tài)、分布、總數(shù)、位置及乳腺局限性不對稱致密。結果①乳腺良性疾病鈣化形態(tài)多粗大(31例),多以散在分布為主(35例),且鈣化總數(shù)較少,較少伴有乳腺局限性不對稱致密影(7例),較少伴有同側腋窩淋巴結的改變(2例)。②乳腺癌的鈣化多以細砂樣鈣化為主(32例),顆粒細小,但鈣化大小不一致,形態(tài)各異,其鈣化的分布多呈簇樣分布(24例),且惡性鈣化的總數(shù)較多,多伴有乳腺局限性不對稱致密影(24例),伴有腋窩淋巴結改變(10例)。兩者鈣化的形態(tài)、分布及總數(shù)的差異均有統(tǒng)計學意義(P lt;0.05)。結論良、惡性乳腺腫瘤的鈣化各有特征,在早期乳腺腫瘤的鑒別診斷中有著重要的臨床意義。
