目的  系統(tǒng)評(píng)價(jià)早期浸潤(rùn)性乳腺癌保乳（breast-conserving therapy，BCT）與全乳腺切除手術(shù)（mastectomy therapy，MT）的遠(yuǎn)期療效。方法  采用Cochrane系統(tǒng)評(píng)價(jià)方法, 檢索Cochrane圖書館臨床對(duì)照試驗(yàn)數(shù)據(jù)庫(kù)、PubMed、EMBASE、Cancer Lit等數(shù)據(jù)庫(kù)，2名評(píng)價(jià)員獨(dú)立提取信息并評(píng)價(jià)文獻(xiàn)質(zhì)量后交叉比對(duì)，評(píng)價(jià)指標(biāo)包括隨訪結(jié)束時(shí)死亡人數(shù)、局部復(fù)發(fā)率及總復(fù)發(fā)率，采用RevMan 4.2.2軟件進(jìn)行Meta分析。結(jié)果  共有6篇文獻(xiàn)進(jìn)入Meta分析，累積樣本含量3 933例。BCT組與MT組隨訪結(jié)束時(shí)死亡人數(shù)差異無(wú)統(tǒng)計(jì)學(xué)意義（OR=1.05，95% CI： 0.93～1.19，P=0.45），而BCT組局部復(fù)發(fā)率（OR=1.64，95% CI： 1.10～2.44，P=0.01）及總復(fù)發(fā)率（OR=1.42，95% CI： 1.22～1.64，P lt;0.01）明顯高于MT組。結(jié)論  對(duì)于早期浸潤(rùn)性乳腺癌，行BCT與MT患者長(zhǎng)期預(yù)后相當(dāng)，但BCT患者術(shù)后局部復(fù)發(fā)率明顯升高，需引起注意。
