• 貴陽(yáng)醫(yī)學(xué)院附屬醫(yī)院肝膽外科（貴陽(yáng)550004）;

目的　 探討不同方式膽道支架置入在惡性梗阻性黃疸患者臨床治療中的應(yīng)用價(jià)值及選擇。
方法 　回顧分析42例不能根治切除的惡性梗阻性黃疸患者采用記憶金屬合金膽道內(nèi)撐支架經(jīng)開(kāi)腹手術(shù)置入(n=18)、經(jīng)PTCD置入(n=17)及經(jīng)ERCP置入(n=7)治療的情況。
結(jié)果 　所有患者均完成膽管末端支架置入達(dá)到內(nèi)引流。支架植入術(shù)后膽紅素均在術(shù)后第1周下降約100 μmol/L； 谷草轉(zhuǎn)氨酶、谷丙轉(zhuǎn)氨酶、堿性磷酸酶及谷氨酰轉(zhuǎn)肽酶在術(shù)后第1周開(kāi)始較術(shù)前明顯下降(P lt;0.05)。全組中位生存時(shí)間為22周，平均生存（32.89±33.87）周。全組圍手術(shù)期死亡2例，死亡率為4.76%。主要并發(fā)癥有膽管炎8例，膽道出血3例，肝功能衰竭2例。
結(jié)論 　膽道支架經(jīng)開(kāi)腹手術(shù)置入、經(jīng)PTCD置入及經(jīng)ERCP置入均可有效解除惡性梗阻性黃疸患者膽管梗阻，改善肝功能； 臨床應(yīng)根據(jù)患者的局部及全身情況選擇不同的置入方式，提高安全性及有效性，盡可能減少并發(fā)癥的發(fā)生。
