目的  研究離體肝臟缺血再灌注早期絲裂原活化蛋白激酶(mitogen activated protein kinase,p38MAPK)信號(hào)轉(zhuǎn)導(dǎo)途徑對(duì)腫瘤壞死因子-α(tumor necrosis factor-α,TNF-α) mRNA和細(xì)胞間黏附分子-1(intercellular adhesion molecule 1,ICAM1) mRNA表達(dá)的影響。方法  建立兔離體肝臟缺血再灌注模型,對(duì)照組(n=12): 灌注液中不加特異性p38MAPK抑制劑SB202190; 抑制組(n=12): 灌注液中加入SB202190(濃度3 μmol/L)。分別于肝臟離體前,冷保存末,再灌注10、30、60及120 min時(shí)獲取肝組織標(biāo)本。分別應(yīng)用Western blot法及免疫沉淀法檢測(cè)肝組織中p38MAPK蛋白的表達(dá)及活性; RT-PCR法檢測(cè)肝組織中TNF-α mRNA的表達(dá)水平,原位雜交法檢測(cè)肝組織中ICAM1 mRNA的表達(dá)水平。結(jié)果  2組動(dòng)物肝組織中p38MAPK蛋白的表達(dá)水平各時(shí)相均無明顯改變(P>0.05),且2組間其表達(dá)水平的差異也無統(tǒng)計(jì)學(xué)意義(P>0.05)。對(duì)照組肝組織中p38MAPK的活性在冷保存末及再灌注10、30及60 min時(shí)均較離體前和再灌注120 min時(shí)明顯升高(P<0.01),也明顯高于同時(shí)相的抑制組(P<0.01); 抑制組p38MAPK活性各時(shí)相的變化差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。離體前、冷保存末及再灌注10及30 min,2組的肝組織中均僅有少量TNF-α mRNA和ICAM1 mRNA表達(dá),組間及組內(nèi)比較差異均無統(tǒng)計(jì)學(xué)意義(P>0.05); 至再灌注60及120 min,2組TNF-α mRNA和ICAM1 mRNA的表達(dá)水平均明顯升高(P<0.05,P<0.01),但抑制組相應(yīng)時(shí)相的表達(dá)水平明顯低于對(duì)照組(P<0.01)。離體再灌注期間肝組織中p38MAPK的活性與TNF-α mRNA及ICAM1 mRNA的表達(dá)水平呈正相關(guān)(r=0.996,P<0.01; r=0.985,P<0.01)。結(jié)論  p38MAPK可能是在轉(zhuǎn)錄水平對(duì)TNF-α和ICAM1的生成發(fā)揮調(diào)節(jié)作用,p38MAPK信號(hào)轉(zhuǎn)導(dǎo)途徑對(duì)TNF-α mRNA和ICAM1 mRNA的調(diào)節(jié)可能是導(dǎo)致離體肝臟缺血再灌注損傷的重要機(jī)理之一。
