目的  總結(jié)重癥急性胰腺炎(SAP)時腸屏障功能障礙(IBD)的發(fā)病機(jī)理、檢測方法及治療進(jìn)展。
方法 復(fù)習(xí)近年來國內(nèi)、外有關(guān)SAP 時IBD方面的文獻(xiàn)并進(jìn)行綜述。
結(jié)果  SAP時IBD的發(fā)病機(jī)理涉及多個環(huán)節(jié),缺血/再灌注損傷、內(nèi)毒素、炎癥介質(zhì)、胃腸激素等起關(guān)鍵作用。IBD的檢測有多種方法,尿乳果糖/甘露醇比值和血漿二胺氧化酶是相對理想的指標(biāo)。積極治療SAP,維持腸道灌注是治療IBD的基礎(chǔ),并在此基礎(chǔ)上合理運(yùn)用促胃腸動力藥、營養(yǎng)支持、中藥等治療。
結(jié)論  IBD是一個復(fù)雜的病理生理過程,近年來大量動物實(shí)驗(yàn)和臨床研究結(jié)果為其的防治提供了新思路,但具體參與其中的細(xì)胞、分子的作用機(jī)理有待于進(jìn)一步研究。
