目的 研究高脂血癥對孕期大鼠急性胰腺炎發(fā)生的影響,并探討其可能機(jī)理。方法 72只妊娠大鼠按抽簽法分為高脂飲食組和均衡飲食組,分別喂養(yǎng)16 d后,用L-精氨酸鹽酸鹽溶液(250 mg/100 g)進(jìn)行腹腔內(nèi)注射,1 h后再次腹腔內(nèi)注射(200 mg/100 g)。分別于末次注射后0、12、18、24、30和48 h各取6只大鼠檢測血清甘油三酯、淀粉酶和脂肪酶,計算胰腺濕/干重比值,觀察胰腺組織的病理變化并評分,免疫組化SP法檢測胰腺組織TNF-α表達(dá)。結(jié)果 各時相高脂飲食組甘油三酯均較均衡飲食組明顯升高(P<0.05); 高脂飲食組淀粉酶及脂肪酶的峰值分別在24 h及18 h出現(xiàn),均衡飲食組延至30及24 h時出現(xiàn)且明顯較高脂飲食組水平低(P<0.05); 在L-精氨酸鹽酸鹽溶液末次注射后12、18和24 h,高脂飲食組的胰腺濕/干重比值較均衡飲食組明顯升高(P<0.05),TNF-α表達(dá)也更高(P<0.05); 在0、12、18及24 h,高脂飲食組胰腺水腫出血、炎癥浸潤更重,病理評分更高(P<0.05)。結(jié)論 高脂血癥能使L-精氨酸誘導(dǎo)的孕期急性胰腺炎提前出現(xiàn),后果更嚴(yán)重,可能是胰腺炎發(fā)生、發(fā)展的一個危險因素,其機(jī)理可能是高脂血癥時甘油三酯大量釋放游離脂肪酸,上調(diào)TNF-α的表達(dá)。
