目的 評(píng)價(jià)早期反復(fù)間斷靜脈-靜脈血濾（repeated intermittent veno-venous hemofiltration，RIVVH）治療重癥急性胰腺炎（severe acute pancreatitis，SAP）的價(jià)值。方法 65例SAP患者隨機(jī)分為RIVVH組(35例)和對(duì)照組(30例)。比較2組患者的生命體征、臨床表現(xiàn)、血糖、氧合指數(shù)、肝腎功能、淀粉酶、脂肪酶、C-反應(yīng)蛋白的變化、細(xì)菌培養(yǎng)陽性率、治療前及治療后的Balthazar CT評(píng)分、APACHE Ⅱ評(píng)分、開腹手術(shù)率、并發(fā)癥發(fā)生率、死亡率、治愈率、平均住院時(shí)間以及費(fèi)用。結(jié)果 RIVVH組35例中2例死于MODS，死亡率為5.7%，對(duì)照組死亡8例（26.7%），其差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）； RIVVH組治愈率為94.3%（33/35），優(yōu)于對(duì)照組的73.3%（22/30），P＜0.05。RIVVH組治療后患者的生命體征、腹部體征、血生化指標(biāo)、氧合指數(shù)和C-反應(yīng)蛋白水平均明顯趨于改善，與對(duì)照組比較其差異有統(tǒng)計(jì)學(xué)意義（P＜0.05，P＜0.01）； RIVVH組Balthazar CT和APACHE Ⅱ評(píng)分較對(duì)照組顯著降低（P=0.002,P＜0.001）； 感染發(fā)生率也較對(duì)照組顯著降低（P＜0.001）。在縮短平均住院時(shí)間，降低平均治療費(fèi)用、開腹手術(shù)率和并發(fā)癥發(fā)生率方面RIVVH組明顯優(yōu)于對(duì)照組（P＜0.05）。結(jié)論 早期RIVVH治療SAP有望成為SAP重要的輔助治療措施。
