目的探討壺腹周圍癌淋巴結(jié)微轉(zhuǎn)移的檢測及其意義。
方法采用免疫組化法檢測60例壺腹周圍癌根治術(shù)患者HE染色陰性的淋巴結(jié)(220枚)中CK7、CK18及CK19的表達(dá)，并結(jié)合臨床資料及隨訪結(jié)果進(jìn)行分析。
結(jié)果60例患者中有17例共43枚淋巴結(jié)有微轉(zhuǎn)移； CK19、CK7及CK18的微轉(zhuǎn)移檢出率分別為19.55%（43/220）、14.55%（32/220）及11.36%（25/220）； CK19的微轉(zhuǎn)移檢出率高于CK7和CK18（P＜0.05）； CK7和CK19的檢出率在臨床Ⅲ、Ⅳ期高于Ⅰ、Ⅱ期（P＜0.05）； 有微轉(zhuǎn)移者比無微轉(zhuǎn)移者的１年生存率低(P＜0.05)。
結(jié)論壺腹周圍癌淋巴結(jié)微轉(zhuǎn)移可能是腫瘤轉(zhuǎn)移的早期階段。CK抗體是檢測壺腹周圍癌淋巴結(jié)微轉(zhuǎn)移的有效指標(biāo),可為臨床綜合治療與預(yù)后判斷提供理論依據(jù)。
