目的探討胃腸道間質瘤(GIST)的診斷與治療。
方法對我院1999年1月至2004年12月收治的48例GIST患者的病理特點及外科治療進行回顧性分析。
結果本組GIST發(fā)生部位: 胃29例(60.4%),小腸11例(22.9%)，結腸3例(6.3%),直腸4例(8.3%),肛管1例(2.1%)。主要臨床表現(xiàn)為消化道出血(52.1%)和腹部腫塊(35.4%)，免疫組化表型CD117陽性率為83.3%(40/48),CD34陽性率為77.1%(37/48),SMA陽性率為27.1%(13/48),S100陽性率為22.9%(11/48)。48例均行手術切除。全組術后均未行放、化療，隨訪12~60個月，平均35個月，隨訪率為100%。良性及交界性患者均無復發(fā)； 13例惡性者中6例復發(fā)，其中5例死亡。
結論GIST在中老年人中好發(fā), 部位以胃及小腸最多見，以消化道出血、腹部腫塊表現(xiàn)為主。腫瘤的大小是GIST良、惡性的重要臨床指標。GIST的診斷有賴于病理形態(tài)學檢查與免疫組化的結合。完整的局部手術切除是最有效的治療手段。
