目的 探討低位直腸癌保留神經(jīng)的腹膜外側(cè)方擴(kuò)大清掃對(duì)患者術(shù)后生存率、排尿功能和性功能的影響。方法 回顧性分析我院1996年1月至2000年6月期間收治的392例進(jìn)展期低位直腸癌實(shí)施保留神經(jīng)的根治性切除術(shù)患者的臨床資料，其中行腹腔內(nèi)清掃173例，腹腔內(nèi)加腹膜外側(cè)方清掃219例，2組患者在年齡、性別、浸潤(rùn)腸壁深度及腫瘤病理組織學(xué)類型方面差異無統(tǒng)計(jì)學(xué)意義。結(jié)果 腹腔內(nèi)加腹膜外側(cè)方清掃組側(cè)方淋巴結(jié)轉(zhuǎn)移率為17.8%（39/219），側(cè)方盆壁非連續(xù)性癌灶轉(zhuǎn)移率為5.9%（13/219）。術(shù)后發(fā)生排尿功能障礙： 腹腔內(nèi)清掃組7例（4.0%），腹腔內(nèi)加腹膜外側(cè)方清掃組113例（51.6%），2組比較差異有統(tǒng)計(jì)學(xué)意義（P＜0.01）； 性功能障礙： 腹腔內(nèi)清掃組93例男性患者中有12例（12.9%），腹腔內(nèi)加腹膜外側(cè)方清掃組119例男性患者中有62例（52.1%），2組比較差異有統(tǒng)計(jì)學(xué)意義（P＜0.01）； 局部復(fù)發(fā)率： 腹腔內(nèi)清掃組為16.2%（28/173），腹腔內(nèi)加腹膜外側(cè)方清掃組為9.6%（21/219），2組比較差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）； 5年生存率： 腹腔內(nèi)清掃組為49.1%（85/173），腹腔內(nèi)加腹膜外側(cè)方清掃組為59.4%（130/219），2組比較差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）。結(jié)論 低位直腸癌保留神經(jīng)的腹膜外側(cè)方擴(kuò)大清掃，可以減少局部復(fù)發(fā)，提高患者5年生存率，但也會(huì)影響患者術(shù)后排尿和男性性機(jī)能。
