【摘要】目的分析不同部位、分期的結(jié)直腸癌及正常黏膜組織中人乳頭狀瘤病毒(human papilloma virus,HPV)16型 E7的表達狀況。
方法應用PCR及免疫組化方法檢測82例手術(shù)切除的結(jié)直腸癌新鮮標本及距腫瘤近端10 cm以遠的正常黏膜組織中HPV16 E7 DNA及蛋白的表達。
結(jié)果結(jié)直腸癌組織HPV16 E7 DNA表達陽性率為51.22%(42/82),明顯高于正常黏膜組織的4.88%(4/82),P<0.01; 直腸癌組織中HPV16 E7 DNA表達陽性率為64.10%(25/39),明顯高于升結(jié)腸癌的18.18%(2/11), P<0.05; HPV16 E7 DNA表達陽性率與Dukes分期有關(guān)(P<0.01),與癌組織分化程度無關(guān)。免疫組化染色顯示,HPV16 E7蛋白主要為細胞核表達,胞漿也可見少量表達,進一步確認了HPV16的感染,且HPV16 E7蛋白與HPV16 E7基因的表達具有相關(guān)性,免疫組化敏感性較PCR方法為低。
結(jié)論結(jié)直腸癌組織HPV16 E7 DNA及蛋白的表達陽性率明顯高于正常黏膜組織,提示部分結(jié)直腸癌存在HPV16感染。癌灶部位距肛門越近,感染率越高; Dukes分期越晚感染率越高。
