【摘要】目的觀察腹膜后自體脾移植聯(lián)合食管下段橫斷術(shù)治療肝硬變門靜脈高壓癥的臨床效果。
方法將20例肝功能Child A、B級(jí)的肝硬變門靜脈高壓癥患者隨機(jī)均分為自體脾移植組和切脾組。自體脾移植組采用自體帶蒂脾組織腹膜后移植聯(lián)合改良的食管下段橫斷術(shù)，切脾組則采用脾切除聯(lián)合改良的食管下段橫斷術(shù)。以患者術(shù)前的情況為對(duì)照，在術(shù)后2～6個(gè)月觀察患者的一般情況、脾掃描、肝功能、血清促吞噬素（tuftsin）及IgM水平。
結(jié)果術(shù)后第6天切脾組死亡1例，術(shù)后第10天脾移植組出現(xiàn)再出血1例。自體脾移植組術(shù)后血清tuftsin、IgM水平高于切脾組，差異有顯著性意義（P＜0.01），而對(duì)肝功能無(wú)明顯影響。
結(jié)論腹膜后自體脾移植能維持脾臟的基本免疫功能，且能長(zhǎng)期存活，在臨床上推廣應(yīng)用是可行的。
