目的 研究社區(qū)感染肺隱球菌病的特點(diǎn)。方法 回顧性分析2003年1月～2007年12月在復(fù)旦大學(xué)中山醫(yī)院住院的30例肺隱球菌病患者的臨床資料。結(jié)果 30例患者中，男19
例，女11例；年齡16～70歲，平均(44±12.64)歲。66.7％ 無(wú)明顯基礎(chǔ)疾病，HIV抗體均I綢性。30％無(wú)癥狀，70％有以下表現(xiàn)：干咳(16.7％)、咳痰(46.7％)、發(fā)熱(23.3％)、胸痛(13.3％)、胸悶（13.3%）。癥狀輕，體征不明顯，有一定自限性。影像學(xué)表現(xiàn)：53.3％表現(xiàn)為單發(fā)或多發(fā)的結(jié)節(jié)、團(tuán)塊影，40％ 以滲出、實(shí)變?yōu)橹?，余為彌漫性混合病灶。病理學(xué)改變：96.7％見(jiàn)肉芽腫性病變，其中93.1％多核巨細(xì)胞胞漿內(nèi)外見(jiàn)透亮空泡樣隱球菌孢子，過(guò)碘酸雪夫(PAS)和Grocott六胺銀(GMS)染色陽(yáng)性率皆為89.7％；3.3％為慢性炎癥。活檢肺組織涂片或痰涂片找到新生隱球菌孢子各1例(3．3％)，活檢組織培養(yǎng)或痰培養(yǎng)新生隱球菌陽(yáng)性率分別為16.7％和3.3％。15例行隱球菌乳膠凝集試驗(yàn)，陽(yáng)性率93.3％ 。結(jié)論 社區(qū)感染肺隱球菌病多發(fā)生于健康狀況較好的人群，臨床表現(xiàn)輕微，影像學(xué)表現(xiàn)多樣，病理學(xué)及相應(yīng)病原學(xué)、血清學(xué)檢查有較高的診斷價(jià)值。
