目的  探討胃竇部癌合并2型糖尿病患者采取不同消化道重建方式后對患者血糖的影響。
方法  回顧性分析我院2006年1月至2012年1月期間行根治性手術(shù)治療的51例胃竇部癌合并2型糖尿病患者的臨床資料,根據(jù)消化道重建方式不同分為BillrothⅠ式組(14例)、Billroth Ⅱ式組(28例)及Roux-en-Y組(9例) 3組,對比分析3組以下指標(biāo):①比較3種重建方式患者術(shù)前、術(shù)后1個(gè)月及術(shù)后6個(gè)月的空腹血糖(FBG)、餐后2h血糖(PG2h)水平;②術(shù)前、術(shù)后6個(gè)月糖化血紅蛋白(HbA1c)的水平;③3組糖尿病的控制情況。
結(jié)果  Billroth Ⅰ式組術(shù)后1個(gè)月和術(shù)后6個(gè)月的FBG、PG2h水平與術(shù)前比較差異均無統(tǒng)計(jì)學(xué)意義(P>0.05),Billroth Ⅱ式組和Roux-en-Y組術(shù)后1個(gè)月和術(shù)后6個(gè)月的FBG、PG2h水平均明顯低于術(shù)前(P<0.05);術(shù)后1個(gè)月和術(shù)后6個(gè)月的FBG、PG2h水平,Billroth Ⅱ式組和Roux-en-Y組間比較,差異均無統(tǒng)計(jì)學(xué)意義(P>0.05),但Billroth Ⅱ式組和Roux-en-Y組均明顯低于Billroth Ⅰ式組(P<0.05)。Billroth Ⅰ式組術(shù)前與術(shù)后6個(gè)月間的HbA1c水平比較,差異無統(tǒng)計(jì)學(xué)意義(P>0.05);Billroth Ⅱ式組和Roux-en-Y組術(shù)后6個(gè)月HbA1c值均明顯低于術(shù)前(P<0.05)。術(shù)后6個(gè)月,Billroth Ⅱ式組和Roux-en-Y組的HbA1c值均分別明顯低于Billroth Ⅰ式組(P<0.05);Billroth Ⅱ式組和Roux-en-Y組間的HbA1c值比較,差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。Billroth Ⅱ式組和Roux-en-Y組患者總體療效均分別明顯優(yōu)于Billroth Ⅰ式組患者(P=0.000,P=0.000),Billroth Ⅱ式組和Roux-en-Y組間比較差異均無統(tǒng)計(jì)學(xué)意義(P=0.259)。
結(jié)論  根據(jù)本組有限病例資料的研究表明,Billroth Ⅱ式吻合及Roux-en-Y吻合對于胃竇部癌合并2型糖尿病的患者可能是最佳的手術(shù)方式。
