目的  探討機(jī)械性抽吸聯(lián)合支架植入治療急性髂、股靜脈血栓和閉塞的臨床效果及應(yīng)用價(jià)值。
方法  筆者所在醫(yī)院自2004年10月至2011年12月期間,收治了273例急性髂、股靜脈血栓合并髂靜脈狹窄或閉塞患者,左側(cè)235例,右側(cè)38例。局麻下經(jīng)皮穿刺患側(cè)股靜脈入路,對(duì)髂、股靜脈血栓,在導(dǎo)絲引導(dǎo)下插入8~14F鞘管行機(jī)械性抽吸,其中53例插入18~24F特大鞘管行機(jī)械性抽吸大塊機(jī)化性血栓。對(duì)股、腘靜脈血栓,經(jīng)健側(cè)股靜脈在導(dǎo)絲抓捕技術(shù)輔助下,置入翻山鞘管至患側(cè),在鞘管支撐下導(dǎo)絲輕柔地越過(guò)瓣膜插至患側(cè)腘靜脈處,沿導(dǎo)絲置入Fogarty球囊導(dǎo)管,在數(shù)字減影血管造影(DSA)透視下順應(yīng)靜脈瓣膜方向?qū)⒀ɡ粱紓?cè)髂靜脈內(nèi),再經(jīng)患側(cè)股靜脈進(jìn)行機(jī)械性抽吸血栓。在髂靜脈血栓機(jī)械性清除后,對(duì)造影顯示的血管狹窄或閉塞段,再進(jìn)行經(jīng)皮血管腔內(nèi)成形術(shù)(percutaneous transluminal angioplasty,PTA)和支架植入。
結(jié)果  血栓完全清除(Ⅲ級(jí))219例(80.22%),部分清除(Ⅱ級(jí))49例(17.95%),清除最少(Ⅰ級(jí))5例(1.83%)。支架植入后,治愈235例(86.08%),顯效29例(10.62%),好轉(zhuǎn)7例(2.57%),無(wú)效2例(0.73%)。總有效率為99.27%(271/273),平均住院時(shí)間7.5d,患肢腫脹和疼痛均于介入治療后1~2d內(nèi)開(kāi)始消退或減輕。隨訪療效:3~6個(gè)月血流通暢259例(94.87%);7~12個(gè)月血流通暢254例(93.73%),13~24個(gè)月血流通暢236例(87.08%),25~36個(gè)月血流通暢228例 (84.13%)。
結(jié)論  機(jī)械性抽吸聯(lián)合支架植入治療急性深靜脈血栓形成和髂靜脈閉塞,療效顯著、病程短,是安全有效的方法。
